Schedule: Monday-Friday, 8:00am-4:30pm, 3 days per week, and 11:30am-8:00pm, 2 days per week.
Transform Diabetes Care Pod RN is responsible for engaging telephonically with members, to present actionable steps to address diabetes gaps in care, evaluate their medical needs and facilitate the member s overall wellness. The Transform Diabetes Care Pod RN will address social determinants of health issues at root cause of care gap, facilitate care gap closure by providing education, connection to resources, and referrals to the member s care management program for ongoing support.
Develop, implement, support, and promote Health Services strategies, tactics, policies, and programs that drive the delivery of quality healthcare to establish competitive business advantage for Aetna. Health Services strategies, policies, and programs are comprised of utilization management, quality management, network management and clinical coverage and policies. Utilizes advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare through assessment and care planning, direct provider coordination/collaboration, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal, cost-effective outcomes.
RN with current unrestricted state licensure required, licensure in all states preferred
Willingness to obtain additional state licenses and CDCES upon hire; Financial assistance available
Strong written and verbal communication skills
3+ years clinical practice experience in chronic condition support
